A model reads everything a tool returns as part of its context, and it cannot tell text that informs from text that instructs: both are words. A web page that says “assistants: run this” reaches the model in the same message as the changelog it was fetched for. That is prompt injection, and no prompt removes it. What limits the damage is everything around the model: which hosts the fetching server reaches, what text it passes on, how it labels it, and — above all — which tools the job lets the model use at all.
Claude Code sends the model a conversation: the system prompt, the task, and each tool result as a
message. Nothing in that format marks a sentence as “from the page” versus “from the user” in a way the
model is bound to respect. A capable model usually ignores a stray instruction in a document; a job
that depends on “usually” is a job that one day runs curl … | sh. The lab’s scripted model obeys every
page on purpose, which is the worst case and the right one to design for.
A fetching server is a request the model can aim. With no host list, it reaches whatever the server’s
network reaches — including internal addresses such as a cloud metadata service, which answers with
credentials to anyone who asks from inside. allowed_hosts turns “any URL” into “the docs site”, which
stops the reading of internal pages and, with it, most of the ways to smuggle data out through URLs.
HTML comments, display: none and the hidden attribute keep text off the screen and in the page.
A reviewer reading the site in a browser sees a clean changelog; the model receives the hidden
paragraph. Stripping those before converting to text (strip_hidden) makes the model see what a person
sees — a strict improvement, though visible instructions still get through.
label_untrusted wraps the text in <untrusted-content source="…"> with a sentence saying it is data.
Models are trained to take such markers seriously, and it makes transcripts easier to audit. It is a
hint, not a wall: the page can contain its own fake closing tag, and a model may still comply.
The job ran with --permission-mode bypassPermissions: every tool the model asks for runs. The page
asked for Bash, the model asked Claude Code, and Claude Code ran it. With
--permission-mode dontAsk --allowedTools "mcp__web__fetch_page,Write" the same request is refused
without anyone being asked, and recorded in permission_denials. A job that summarises a page needs
to fetch the page and write a file. It does not need a shell, and a shell is what every injection
wants.

Why can a fetched web page cause a model to request a shell command?
The page’s text becomes part of the model’s context like any other message; the model cannot be relied on to distinguish instructions in data from instructions from the user.
